As someone who had my first iPhone app completely stonewalled by Apple's review process until Steve Jobs himself intervened to get it into the App Store, I've done quite a bit of thinking about how to responsibly circumvent their self-interested monopoly. One thing I thought about doing was making my app open source so that anyone who wanted to could audit, build, and install it on their own device. Apple already has mechanisms in place (e.g., Apple Business Manager, TestFlight, etc.) that allow developers to distribute apps "directly", presumably without overly compromising their security. I see no reason these methods could not be improved or extended.
